Entry Form Requirements

I decided for the form that I needed the following boxes to be completed to gain the required information:

• Name

• Email

• Telephone

• Mobile

• Address

• Vehicle Make and Model

• Year of Manufacture

• Vehicle type and description (brief description for programme)

This should allow me to complete the spreadsheet that has been set up to keep track of entrants into the show.
